Good afternoon.  I hope everyone is staying warm in this chilly weather.  I wanted to give you the wonderful Covid-19 updates, but also share a few miscellaneous items.  

As Christmas quickly approaches, the ladies in the Activity Department are planning our visit from Santa.  He will be here on December 17th to pass out gifts to the Elders.  With that being said, if you would like to bring a gift for Santa to give to your loved one, please bring it to the Administrative Building or Villa 6 (where most managers are now located). They are asking that you limit the number of gifts (for Santa to give) to one present.  Of course, you are more than welcome to give your loved one as many gifts as you would like, during your private time with them.  Our goal is to make sure all Elders receive the same number of gifts from Santa. We certainly don't want Elders to think he has favorites.  ðŸ˜Š  The deadline for the gifts will be Wednesday, December 15th.  Again, if you would like to bring something, please drop them off at the Administrative Building or Villa 6.

On another note, the nurse managers have reported a lot of over-the-counter (OTC) medications in the resident rooms.  Due to regulations we operate by, we are required to follow certain guidelines for any medication that is brought in or used.  If you are providing any OTC or Rx medications, please make sure you give them directly to the nurse on duty.  If we don't have a physician's order, the nurse can call and request one.  (All medications and treatments require a physician's order and monitoring).  The nurse will then create a label for the medications, including the directions of use per the MD, the name of the Elder, date of birth, etc.  We then are required to do an assessment to ensure the Elder is safe to keep it at bedside/in their room.  This is for their safety and well-being.

1.                 EXCURSIONS & APPOINTMENTS – 

•             Medical Appointments / Excursions Greater Than 24 Hours:  For all medical appointments and for excursions greater than 24 hours, elders must be screened and tested with a point-of-care (POC) test immediately upon return and must also be tested with a POC test 3-5 days following the elder's return. 

•             Excursions Less Than 24 Hours:  If the excursion is less than 24 hours and is not a medical appointment, the elder must be screened upon return and only needs to be tested with a POC test immediately and within 3-5 days if the elder experienced a high-risk exposure.

Excursions and non-essential appointments put elders at significant risk, regardless of your elder's vaccination status.  Please consider refraining from taking your elder on excursions altogether.  Likewise, please consider turning over to Guerin Woods the task of making all of your elder's appointments, medical or otherwise.  This will allow us to minimize their exposure to infection by arranging telehealth or in-room treatment, as appropriate. 

a.      If – despite the risk – you take your elder out for an excursion or appointment, please provide Guerin Woods advance notice, if at all possible, via our scheduling application, Simply Book ME.  Click Here to Schedule Elder Excursion or Appointment

b.      When elders return from an excursion or appointment, we are now required to screen and test them before they return to their room per the guidelines set forth above

c.       If an elder is required to be tested upon their return to campus, they will be required to wait with you in the parking lot until:

i.        A nurse is available to perform screening and a COVID-19 test;

ii.      The results of the COVID-19 test are available (15 – 30 minutes); and

iii.     The nurse establishes that the COVID-19 test result is negative.

d.      Elders who leave campus for an excursion or appointment will also be required to undergo a second test between the third and fifth day after their return.

e.      Unvaccinated elders who are away from campus for 24 hours or more are now required to be in isolation for no fewer than 14 days upon their return, even if COVID-19 testing is negative and the elder has no symptoms.

2.                 VISITOR SCREENINGALL VISITORS MUST COMPLETE A SCREENING FORM AND THE FORM MUST BE SIGNED BY A STAFF MEMBER.  YOU MAY NOT PROCEED TO YOUR ELDER'S ROOM UNTIL A STAFF MEMBER REVIEWS AND SIGNS YOUR SCREENING FORM! 

3.                 EFC PROGRAM: We strongly encourage visitors to apply to become Essential Family Caregivers (EFCs).  In most circumstances, EFCs will be allowed to continue to visit in "YELLOW" Villas, albeit under rules that are more strict, should normal visitation be prohibited. If you have served as an EFC in the past, please re-apply; the Department of Health has altered its guidance and you will need to sign a new acknowledgment.  Below is a short summary of the EFC Program.  If you have any questions, please contact me.  I have attached the EFC Application to this email

b.      Purpose:  The purpose of the EFC Program is to allow family members or other caregivers to continue to provide routine care during a public health emergency pursuant to state guidance. 

c.       State Requirements

i.        Each EFC applicant must be at least 18 years old; and

ii.      The applicant must have regularly engaged with the elder providing supportive or direct care (e.g., help with meals, grooming, or companionship), on average twice weekly, prior to the start of the public health emergency;

iii.     The Elder or POA must approve the EFC;

iv.     The EFC must be able and willing to (a) undergo COVID-19 testing on the same frequency as Guerin Woods staff and (b) follow all precautionary measures, such as hand hygiene, and use of N95 masks, surgical gowns, goggles, and gloves.

d.      Approval Process:  Guerin Woods will approve or reject EFC applications on a case-by-case basis.  If an application is denied, we will notify the applicant in writing of our reasons and the applicant will have an opportunity to request reconsideration.  When an application is approved:

i.        Guerin Woods will work with the EFC to determine appropriate visitation hours and duration;

ii.      Guerin Woods will provide the EFC all visitation and EFC requirements in writing in the form of an agreement; and

iii.     The EFC must attest that they will abide by the EFC requirements congruent with existing guidelines, including those relating to testing, screening, infection prevention protocols, and approved visitation locations.

e.      Scheduling: Please contact Chelsea Wild regarding the days/times you wish to participate in EFC visitation.

f.        Application/Agreement: Please send your completed EFC Application to me via email, mail, or by leaving a copy at the Administration Building.  If your application is approved, you will receive an electronic copy of the agreement via email.  Please execute it and return a copy to me.

g.      Testing/Training:  Before you begin EFC visitation, please contact Chelsea regarding COVID-19 testing and training.  We need to ensure a nurse if available to facilitate both. EFCs are subject to all of the same testing and PPE requirements as staff.  You will need to arrange for and be tested for COVID-19 twice weekly,on either Monday & Friday or Tuesday & Saturday.

h.      Sign-In Process: As with normal visitation, you will be required to sing-in before your EFC visitation.  You should also record your temperature on the sign-in form in the same way employees do.  Please note that you may be directed to enter the Villa through the rear door.

4.                 "Non-EFC Visitor" Testing:  We strongly encourage you to be tested on a regular basis if you visit the Guerin Woods campus, especially of you are unvaccinated.  If you are unvaccinated, wish to be tested routinely but are unable to do so, please let us know and we will do our best to accommodate you.

5.              VISITATION

A.                RESTRICTIONS- none at this time

B.                 Notification Requirement – NONE, unless utilizing alternative methods of visitation, e.g., window and video visits.  

*Previously, with scheduled visits, our therapists and CNAs were able to schedule around visitation.  Because this will no long be possible, please be prepared to be flexible.   Your visit may be interrupted or need to be postponed for a short period of time if therapy or some other intervention is underway or scheduled during your visit.

C.                 Visitor Screening Form – Prior to moving to an elder's room, ALL VISITORS – INCLUDING EFC VISITORS -- MUST COMPLETE A SCREENING FORM AND THE FORM MUST BE SIGNED BY A STAFF MEMBER.  DO NOT PROCEED TO YOUR ELDER'S ROOM UNTIL A STAFF MEMBER REVIEWS AND SIGNS YOUR SCREENING FORM!  WE CANNOT ALLOW VISITATION IF A VISITOR'S SCREENING RESPONSES OR SYMPTOMS INDICATE A RISK OF EXPOSURE. 

D.                Infection Control

a.                    Hand hygiene – Visitors must adhere to hand hygiene requirements upon entry and during their visit, with the use of alcohol-based hand rub being preferred.  You must continue to clean your hands frequently with either alcohol-based hand rub or soap and water, both of which are readily available in the Villas.  Visitors should clean their hands after touching potentially contaminated surfaces, using soap and water after using the restroom. 

b.                    Surgical Mask (covering BOTH mouth and nose) – All visitors must wear a surgical mask at all times while on the Guerin Woods campus.  (EFC Visitors in a "YELLOW" Villa must wear an N95 respirator, surgical gown, goggles, and glove.)

c.                     Social Distancing – All visits are limited to Elders' rooms and visitors may not be present in common areas other than to make their way to an Elder's room.  Visitors are not permitted to interact with other Elders and must maintain social distancing when communicating with staff.  Unvaccinated visitors must maintain social distancing at all times, both in the common areas and their Elder's room.

E.                 Hours – Daily, 7 a.m. – 7 p.m.

F.                 Location – Visitation is restricted to Elders' rooms.  Families and friends may also use the Villas' garden areas, but only when visiting a "GREEN" Villa.  If a visitor wishes to speak with a staff member, the visitor must use their Elder's call button and may not – under any circumstance -- wander through the common areas.

G.                Number of Visitors – If appropriate social distancing is maintained, Elders may have up to 4 visitors at a time.

H.                Age Restrictions – Because all visitors must be able to comply with infection prevention protocols, Guerin Woods strongly discourages any visitor under the age of 12.  EFC visitors must be 18 or older.

IV.               ELDER TESTING & TRANSFERS: Guerin Woods is following the guidance and directives of the Centers for Disease Control (CDC), Indiana State Department of Health (ISDH), and Floyd County Health Department regarding Elder testing.  If an Elder were to test positive for COVID-19, the Elder would be transferred to a hospital or other appropriate facility.

IV.               ELDER TESTING & TRANSFERS: Guerin Woods is following the guidance and directives of the Centers for Disease Control (CDC), Indiana State Department of Health (ISDH), and Floyd County Health Department regarding Elder testing.  If an Elder were to test positive for COVID-19, the Elder would be transferred to a hospital or other appropriate facility.

Announcement re: the Villas of Guerin Woods

Dear Guerin Woods Families & Friends:

 

On behalf of Guerin Woods’s directors, it is with pleasure --  but also great sadness -- that I announce the transfer of ownership of the Villas, effective December 31, 2021.

 

The pandemic forced us to address our sustainability as an independent provider of assisted living, skilled nursing, and memory care.  When considering proposals from potential purchasers, our primary focus was on finding a partner with both expertise in long-term care and the ability to sustain our mission into the future.  We were blessed with multiple offers, but ultimately decided to sell to an organization whose principals include former long-term care administrators who operate a small number of other facilities in Indiana.

 

What impact will this decision have on elders and staff?

 

For most, very little to none. The owner of the Villas (1002 Sister Barbara Way Property LLC) and the new manager (Villas of Guerin Woods Healthcare LLC) intend for the transition to be seamless, both internally and externally.  They have no interest in changing our unique, small-house model.  The new owners firmly believe the design of the Villas provides the best in elder care and hope to replicate it elsewhere.

 

From the effective date of sale to the following day, operations should not miss a beat, even the name will remain the same.  Although there might be some tweaks, the new owner even intends to use our electronic medical records system (Point Click Care) and timekeeping and payrolls systems (SmartLinx and Viventium).

 

Over the coming weeks, elders, their families, and staff will have an opportunity to personally meet the principals who are investing in the Villas. Their transition team will also be working with Guerin Woods’s leadership throughout the transition process.

 

Attached please find short biographies of the new owner’s principals as well as a list of questions and answers. Should you have any specific questions not addressed in the Q&As, you can leave them in one of the question boxes located in the Villas and the Administration Building.  We will collect your questions, combine them, and respond as soon as possible.

 

We hope you will appreciate that — every step along the way — we considered what was best for our elders, our staff, and the community.  Likewise, we considered carefully whom we wanted as long-term neighbors and how they would build upon Sister Barbara’s vision and the reputation the Villas have earned over the years.

 

While change can be difficult, we hope you will bear with us as we meet the challenges of this transition.

Questions & Answers

 

Staff

 

     Will most of the current staff in place be invited to continue to work with the organization after the transition? Yes, we will be working with the Administrator at Guerin Wood and will obtain her input as well.

     Will staff need to re-apply with the new entity for their positions? We will provide offer letters to the staff prior to transition.

     Will all current staff be invited to continue working/keep their positions after December 31? Yes, that is our intention, and we will also consider the Administrator’s input as well.

     Will there be any changes in salary to retain existing staff? We will honor what the staff is currently making.

     How do you anticipate current staff will react to changes brought upon by this change? We look forward to working with the outstanding Villas leadership team already in place and only anticipate changes that the staff will agree are positive.

     What are the changes, if any, that staff will expect in the first 6 months? In the long run? We hope that staff won’t feel a difference in the short term, and in the long term only positive changes. We will have extra training and leadership for the staff at the facility and they generally see this as a long-term positive change.

     What measures will you take to address the staffing shortage and turnover? It’s very difficult to answer this question without first understanding the unique needs of the Villas. Of course, we will do aggressive recruitment, etc., but we would first need to get “under the hood” of the facility to better understand the needs of the facility.

     What is your plan for attracting and retaining competent and compassionate staff? Again, each demographic is slightly different and the staff from that demographic will dictate to us the answer to this question.

     What professional development and “onboarding” opportunities will you offer? We have a very robust onboarding and HR system. We use the EMTRUST onboarding system, and the staff loves this.

 

ELDERS and FAMILIES 

 

•    What immediate changes should elders expect? We don’t anticipate any.

•    What are the changes, if any, that elders should expect in the first 6 months? In the long run? We don’t anticipate any.

•    Will there be a cost increase? Initially no, but we will do a market study.

•    What will happen to the staff? We hope to retain them and will work with the leadership at the facility.

•    What other facilities do you operate? Are any of them in Indiana? In the Midwest? Yes, we have several homes in Indiana, the closest one being Shady Nook.

•    How will you manage this property from several states away? We will rely on the Villas management team to manage all day-to-day issues, and we have local experts in Indiana to support them. We will also be visiting the Villas on a regular basis.

•    Will we just become a number to you? How will we be treated? Never. We recognize and treat each elder as an individual and each facility as being unique.

•    Change is difficult for elders and particularly those in the small-house setting. How will you address this? Because we don’t anticipate any changes, we do not believe there will be a negative impact on elders.

     What will you do to be proactive about staffing shortages? Will you over-staff to offset staffing needs during the transition and in light of predictions that COVID is with us another twelve months at least? We always do our best to staff our facilities appropriately to give proper care.

 

COMMUNITY    

 

•    How active do you as principals intend to be in the community? Fairly active. We plan to encourage local involvement by staff and anticipate visiting often.

•    Do you anticipate any layoffs after December 31? No.

•    What job creation opportunities do you anticipate? It’s very difficult to answer this question without first understanding the unique needs of the Villas.

•    Will you hold any job fairs in the immediate future? We intend to.

•    Will you hold a meet-n-greet with key community partners? We intend to.

•    Will you hold a grand reopening/open house for the community? We intend to.

•    Can you give us examples of other healthcare ventures you acquired as going concerns that are doing well? Sure! Shady Nook in Indiana. Cherry Hill in Michigan.

•    What are the changes, if any, that the general public will expect from your change in operations and administration? We anticipate only positive changes.

•    How will you partner with members of our local community? It’s very difficult to answer this question without first understanding the unique needs of the Villas.

•    How will you preserve the historic connection with Sisters of Providence, Sister Barbara and the founders of PSSM? We first have to be involved to better understand how to keep the connection.

•    How will you assure the community you are here for the long-term and not just short-term to possibly sell again? As one sign of our commitment, we have agreed to assume the Villa’s affordable housing grant commitments, as originally envisioned by Sister Barbara. We firmly believe that over time the community will see our dedication to this facility and we will earn their trust.

•    What plans do you have for improvements and investments in facilities and grounds? We are not aware of needed improvements and investments at this time, but intend to well maintain the Villas and will assess investments in the facility and grounds over time.

David and Akiva are licensed nursing home administrators with extensive, hands-on experience in operating small, medium, and large-scale facilities, and currently have an ownership interest in numerous rehabilitation and long-term care facilities, including a few in Indiana. Although the Villas have a 5-Star rating, much of their experience has been in improving under-performing long-term health care facilities. David prides himself on assisting facilities implement best-in-class management processes, technology, and human resources practices to ensure superior quality of care for residents. Likewise, Akiva enjoys sharing his experience and educating and sharing best practices in nursing home administration and services. David and Akiva are thrilled to learn about the Villas’ small-house model, as well as the possibility of replicating it elsewhere. They firmly believe it represents the manner in which elders should be cared for.
